Optimize Health is the market leader in remote patient monitoring (RPM). We make it easy for healthcare providers to provide better care for patients in their own homes. We strive to simplify remote care and reduce operating expenses. The purpose is to improve patient outcomes, lower hospitalizations, and reduce patient costs. The Optimize Health platform combines connected devices, data science, healthcare data integration, communication, and local delivery into a single experience. The software is easy to use and is compliant with Medicare's new CPT codes (99453, 99454, 99457, 99091). Optimize Health was founded in 2015 by entrepreneurs Jeff LeBrun and Chuks Onwuneme. Todd Haedrich was appointed CEO in September 2021.
